The assessment associated to this column was in a folder that was deleted. This is a known issue in Blackboard. If you delete a folder containing assessments rather than deleting individual assessments, the deployed assessment will be deleted, but the column in the Grade Center will not be deleted.

Access the cell's menu and select Exempt Grade. You can clear the exemption with the Clear Exemption option in the cell's menu.
In the Grade Center, locate the cell with the student's graded test or assignment grade to override and access its menu. Select View Grade Details. On the Grade Details page, select the Manual Override tab and type a new grade in the Override Grade box. Optionally, type Feedback to User and Grading Notes for yourself.
